What makes Sports and Wellness Physical Therapy different from other
clinics?

We’re owned and operated by a physical therapist, not a big corporation. And
unlike other facilities where you’re treated with several other patients at the same
time, at Sports and Wellness you’ll receive patient-focused one-on-one care.
The goal of our customized approach is to help you feel better as quickly as
possible.

Do I need a prescription for PT?

Some insurance plans require a referral/prescription from an approved medical
provider, such as a doctor, PA, ARNP, dentist, or chiropractor. In Florida, direct
access means you can see a physical therapist for 30 days without a referral/
prescription from your doctor. If you wish to continue your therapy past the 30-
day mark, you will need your medical provider to write a referral to continue your
care. Most insurance companies pay for direct access services, but they vary in
the number of visits they pay for. For those patients who wish to pursue self-
referral without insurance, please contact our staff about payment options.


Do we take insurance?

At Sports and Wellness, we take most commercial plans, Medicare, automobile
accident, and Worker’s Compensation. You can check with your insurance
provider to determine your coverage for physical therapy services, including
direct access. If you do not have physical therapy coverage or lack medical
insurance, we can still help you! Ask about our simple self-pay rate if you wish to
pay for your physical therapy visits out-of-pocket.


Will I have a copay or co-insurance?

Your insurance provider may require you to pay for a portion of your care through
a COPAY (set rate per visit) or CO-INSURANCE (you are responsible for a
percentage of the cost). In addition, many any plans begin the year with a
DEDUCTIBLE (you pay upfront until the deductible is met). We recommend that
you check with your insurance provider to determine your out-of-pocket costs
before starting your treatment.


What should I bring to my first PT visit?


Make sure to bring the physical therapy prescription given to you by your
referring physician. You will also need to bring your medical insurance ID card
and photo ID if you will be using these benefits to help pay for your care. Be sure
to bring a list of all current medications, including other medical conditions. Any
tests or imaging- like X-rays, MRIs and their reports are beneficial. In addition,
any operative report or pictures from your surgery can help us gain specific
insight into your individual condition and aid in treatment planning. If possible, it’s
best to wear loose-fitting, comfortable clothing.
